Tax Forms

Due to recently announced IRS budget cuts, the library will be receiving a very limited number of IRS forms and instruction booklets this year. Please call the Reference Desk (978-897-1010 x103) before coming to the library to make sure we have what you are looking for.
Federal forms and publications are available online at: https://www.irs.gov/forms-instructions
You may order forms to be delivered by U.S. mail at: https://www.irs.gov/forms-pubs/order-products, or by calling 1-800-829-3676.
Massachusetts forms and instructions are available online at: https://www.mass.gov/service-details/dor-tax-forms-and-instructions or by calling the Department of Revenue at 1-800-392-6089.
